Technical Assistance:
Technical assistance was provided regarding the forms required for the children's records. Inspector will email a copy of sample forms for the children's records and for documenting emergency drills.

Comments:
An unannounced monitoring inspection was conducted today from 9:30am-11:00am. There were 3 children (9 points) supervised by 1 direct caregiver. The home, 3 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed engaging freely. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Violations:
Standard #: 22VAC40-111-800-A
Description: Based on review of emergency plans, there isn't a written emergency preparedness and response plan. Evidence: There is no documentation of an emergency evacuation plan, method to alert caregivers of emergency, method to ensure essential documents are included in the evacuation and evacuation procedures.

Plan of Correction: I will put together the plan and inform the parents of the play.

Standard #: 22VAC40-111-830-C
Description: Based on review of emergency plans and interview with provider, there is no documentation of emergency evacuation and shelter-in-place drills. Evidence: Emergency evacuation are not being practiced monthly and shelter-in-place drills are not being practiced twice a year.

Plan of Correction: I will begin having emergency evacuation and shelter-in-place drills.

Standard #: 22VAC40-111-70-A
Description: Based on review of 3 children's records, 2 are incomplete. Evidence: Children #1, 2, and 3 do not have documentation of a parent receiving operating information including the hours, days, schedule of fees and payment plans, check in/out procedures, policy for administration of medications, liability insurance information or any information for the parents.

Plan of Correction: I'm not sure what forms are required, can you send me copies of the forms and I will make sure we get what is required.

Standard #: 22VAC40-111-80-A
Description: Based on review of 3 children's records, 3 do not have documentation of proof of age and identity. Evidence: Children #1, 2, and 3 did not have documentation of proof of age and identity.

Plan of Correction: I will ask the parents and get the information.

Standard #: 22VAC40-111-510-C
Description: Based on observation, not all occupied cribs were at least 12 inches from each other. Evidence: A child was in a crib that was not 12 inches away from another crib.

Plan of Correction: Corrected during inspection, going forward I will make sure the cribs are at least 12 inches apart.
