Technical Assistance:
Had a discussion with the provider regarding implementing academic programming as well as transitional activities. The provider's medication training has expired; provider was reminded that they cannot accept any medication from the families until training has been renewed. The next anticipated inspection will be the provider?s renewal inspection. Provider was reminded to have all renewal application documents completed and sent into licensing office 45 days prior to expiration for processing. The renewal application is available on the VDSS website.

Comments:
Today an unannounced Monitoring inspection took place between 10:30am and 2:00pm. At the start of the inspection there were 11 children present with 2 assistant caregivers for a total of 29 points within the supervision guidelines; the provider arrived at the family day home approximately 15 minutes after the inspection began. The children were observed playing freely, washing hands, eating lunch (brought from home) and napping. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-760-A-1
Description: Based on observation, the first aid kid did not contain the minimum supplies. Evidence: The provider's first aid kit did not contain adhesive tape or two triangular bandages.

Plan of Correction: I will buy new ones.

Standard #: 22VAC40-111-890
Description: Based on observation, food brought from the children's homes were not clearly labeled in a way that identifies the owner. Evidence: 6 out of 7 lunch bags in the provider's refrigerator were not labeled with any children's names. The food containers within the lunch bags also did not contain the children's names.

Plan of Correction: Tomorrow we will start doing it.

Standard #: 22VAC40-111-940-A
Description: Based on observation, the refrigerator temperature is not maintained at or below 40 degrees Fahrenheit. Evidence: The thermometer is the refrigerator reads 52 degrees Fahrenheit.

Plan of Correction: The thermometer only says that because it fell. It was at 35 degrees this morning. The refrigerator temperature is good. I will need to buy a new thermometer.

Standard #: 22VAC40-111-120-B
Description: Based on record review, assistants records did not contain all required information. Evidence: Caregiver #2's record does not contain documentation of an emergency contact.

Plan of Correction: I will fix it by tomorrow.

Standard #: 22VAC40-111-60-B
Description: Based on record review, children's records did not contain all required information. Evidence: Child #2's file did not contain the 'information for parents' form or the provider's liability insurance declaration. Child #3's file did not contain documentation of the families medical insurance policy number.

Plan of Correction: I will tell the parents right away that I need this.

Standard #: 22VAC40-111-180-A
Description: Based on record review, the provider did not obtain a current report of tuberculosis (TB) screening every two years for each caregiver. Evidence: Caregiver #1's most recent TB screening is dated 11/30/2016. Caregiver #3's most recent TB screening is dated 12/1/2016.

Plan of Correction: We will have to make an appointment to get that done.

Standard #: 22VAC40-111-240-A
Description: Based on observation, furnishings of the family day home are not maintained in a clean and safe condition. Evidence: Four out of four cribs in the nap room have chipped and peeling paint along the edges.

Plan of Correction: I will find out how to fix this.

Standard #: 22VAC40-111-480-D
Description: Based on observation, equipment used by children were not used in accordance with the manufacturer's instructions. Evidence: Child #1 was observed sleeping in a mobile swing. The child was no buckled into the swing; they were laying on top of the buckles.

Plan of Correction: This was corrected during the inspection.

Standard #: 22VAC40-111-510-C
Description: Based on observation, occupied cribs, cots and rest mats are not at least 12 inches from each other. Evidence: The head of one occupied rest mat is observed approximately 4 inches from the foot of an occupied crib. The foot of another occupied mat is observed approximately 2 inches from the foot of another occupied mat.

Plan of Correction: I will fix it for tomorrow's nap time.

Standard #: 22VAC40-111-690-D
Description: Based on observation, the diapering surface is absorbent. Evidence: There are two tears on the top of the diaper changing pad making it absorbent.

Plan of Correction: I will buy a new one.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation, the findings of the most recent inspection were not posted. Evidence: There are no inspection's findings posted in the family day home. The most recent inspection summary and findings were in a folder with the provider's records.

Plan of Correction: Today's will get posted.
