63.2 Licensure and Registration Procedures
An unannounced monitoring inspection was completed on 6/4/19. There were 11 children present from eight months through four years with the provider and two assistant caregivers supervising. The point total was 29. Five children's records, one newly hired assistant caregiver's record, and the provider's record were reviewed. The inspector arrived at the provider's home at 11:30 am and departed at 1:30 pm. The children were observe during outside water play time, during inside free play, during lunch, and during nap.
Standard #: 22VAC40-111-90-A Description: Based on record review, the provider failed to ensure that before a child attends the family day home that the provider obtains documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations. Evidence: 1. The immunization record for child #4 was dated 12/4/18 by the physician's designee. The documented first date of attendance for child #4 was 12/3/18. Plan of Correction: The provider will make sure the first date of attendance is the actual date that the child starts and that the child does not start attending prior to obtaining the immunization records.
