Technical Assistance:
We discussed the requirements for outside maintenance. The risk assessment for violations were provided. We reviewed the requirements for emergency practice drills.

Comments:
Thank you for your assistance during the renewal inspection conducted from 11:35 AM to 3:40 PM. Today, there were two school age children in care and six resident children present with the provider. I viewed daily routines, menu, lunch, nutrition, staffing, supervision, interactions with children, pet record, posted information, equipment, activities, transportation and community trip procedures, emergency supplies and drill records, emergency response plan, five records for children, one caregiver record, caregiver training, posted information, indoor and outdoor areas. The children enjoyed playing outside together and had activities in the living room. The provider was attentive to individual children's needs. The provider has participated in ongoing training opportunities. Recommendation is for issuance of a regular license. Let me know if you need any assistance. 540/430-9259

Violations:
Standard #: 22VAC40-111-240-A
Description: Based on observation of the outdoor play area and equipment, and an interview with the provider, the areas and furnishings of the outdoor area have not been maintained in a safe and operable condition. Evidence: Bricks and cinder blocks were accessible on the ground at the front of the storage shed. Four boards, including one with three nails, were accessible on the ground at the back side of the storage shed. A red toddler swing was split and detached on one side of the seat. Four metal support pegs for the music chimes were protruding from the ground three fourth's to one inch high, creating a tripping hazard.

Plan of Correction: The provider arranged with a parent to remove the bricks and boards from the yard. The provider immediately obtained help placing the swing out of reach of the children and will ask the parent to remove the broken swing seat. The provider immediately had the pegs hammered to the ground. In the future, the provider will check the playground area after storms and when the children will use the outdoor areas regularly.
