Discussed with provider best about requesting parents update and review children's files while in the center and not allowing the parents to take required paperwork home.

An unannounced monitoring inspection was conducted today from 10:30am-11:45am. There were 6 children (15 points) directly supervised by the provider and substitute provider. The home, 2 staff records, 5 children?s records, 1 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. A group of children were observed participating in water table play with the provider outside. The substitute was inside while an infant was napping in a crib. The home was supplied with toys and books appropriate for the ages of children in care. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-60-B Description: Based on record review, 1 of 5 children's records reviewed was incomplete. Evidence: Child 4's record did not contain written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately unless the parent presents a written objection to provision of medical treatment on religious or other grounds. Plan of Correction: I will ask the parents to sign the document.
