Technical Assistance:
Discussed with provider best about requesting parents update and review children's files while in the center and not allowing the parents to take required paperwork home.

Comments:
An unannounced monitoring inspection was conducted today from 10:30am-11:45am. There were 6 children (15 points) directly supervised by the provider and substitute provider. The home, 2 staff records, 5 children?s records, 1 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. A group of children were observed participating in water table play with the provider outside. The substitute was inside while an infant was napping in a crib. The home was supplied with toys and books appropriate for the ages of children in care. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice. Description: Based on record review, 1 of 5 children's records reviewed was incomplete. Evidence: Child 4's record did not contain written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately unless the parent presents a written objection to provision of medical treatment on religious or other grounds.

Plan of Correction: I will ask the parents to sign the document.
