Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and licensing questions. One crib view on site (manufacture date - March/2019. Pool on site surrounded by a 5 foot impenetrable fence. One staff has obtained certification for a Water Safety Education for Parents & Caregivers Course.

Comments:
The following inspection took place between the hours of 10 am through 2:30 pm. Today, I met with the Provider on site at the Family Day Home (FDH) located in Lovettsville, Virginia. At the start of this announced intial inspection there were a total of 6 children (13 points) with 3 caregivers. A total of 6 children?s files, 3 staff files, and 3 household member?s files were reviewed. The children were engaged in the following: toy play, games, music, movie time, large motor skills outside in the backyard, lunch, hand washing, diapering, nap time, and other organized activities. Standard #: 22VAC40-111-770
Description: Based on review, a working portable battery-operated weather band radio was not available. Evidence - 1. On the date of inspection (06/07/2019), the Family Day Home (FDH) had a FM radio. 2. It did not appear nor was it listed within the radio's manual that it had a weather band capability.

Plan of Correction: A weather band radio shall be purchased and kept on site.

Standard #: 22VAC40-111-120-A
Description: Based on review, 1 of 3 staff files reviewed indicated that not all of the required information was on file for each caregiver. Evidence - 1. On the date of inspection (06/07/2019), the emergency contact information and 2 references for Staff #2 were not available for review. 2. The start date for Staff #2 was listed as - 03/17/2018

Plan of Correction: An ASSISTANT/SUBSTITUTE PROVIDER RECORD shall be completed for all applicable staff. At least 2 references shall be obtained and placed on file for all applicable staff.

Standard #: 22VAC40-111-60-B
Description: Based on review, 1 of 6 children's files reviewed indicated that not all of the required information/documentation was contained within each child's record. Evidence - 1. On the date of inspection (06/07/2019), 1 of 2 emergency contacts were listed for Child #1. 2. The Proof of Identification was not available for review for Child #6. The start date for Child #6 was listed as 04/02/2019.

Plan of Correction: All missing information/documentation shall be obtained and placed on file.

Standard #: 22VAC40-111-200-A
Description: Based on review, 2 of 2 staff files reviewed indicated that documentation of the orientation training was not available for review. Evidence - 1. On the date of inspection (06/07/2019), documentation of the orientation training for Staff #2, and Staff #3 was not available for review. 2. The start date for Staff #2 was listed as - 03/17/2018, and the start date for Staff #3 was listed as - 02/06/2019.

Plan of Correction: An orientation form shall be completed and placed on file for both staff.

Standard #: 22VAC40-111-240-A
Description: One area of the home, inside was not maintained in a safe condition. Evidence - 1. On the date of inspection (06/07/2019), within the downstairs hallway, had two spots on the wall were the drywall was observed to be peeling. 2. This area was accessible to the children in care and was just off from the area where they slept.

Plan of Correction: Area will be covered to prevent access until it is properly repaired.

Standard #: 22VAC40-191-40-C-1-a
Description: Based on review, a search of the central registry (CPS check) was not available for review for the applicant upon application for licensure. Evidence - 1. On the date of inspection (06/07/2019), a CPS check was not available for review for Staff #1. 2. The copy to be mailed was observed notarized and on file.

Plan of Correction: This request shall be sent to the central registry for their review.

Standard #: 22VAC40-191-40-C-1-c
Description: Based on review, 2 of 2 staff files, and 3 of 3 household member's files indicated that a search of the central registry (CPS check) was not available. Evidence - 1. On the date of inspection (06/07/2019), a CPS check was not available for review for Staff #2, Staff #3, Household Member #1, Household Member #2, and Household Member #3. 2. The copies to be mailed were observed notarized and on file.

Plan of Correction: These requests shall be sent to the central registry for their review.
