Technical Assistance:

As of the date of the inspection a renewal application has not been submitted to the Fairfax Licensing Office with the fee to renew the license prior to the expiration June 25, 2019. Please check your e-mail for the application.

Comments:

An unannounced renewal study was conducted today. Eight children were present with two caregivers for twenty eight points which is within ratios for supervision. Observed children playing on the back deck, hand washing, infant care and transitioning to lunch. Lunch served today was mac and cheese, avocado, sweet potatoes and milk. Description:	Based on records review, the written emergency plan was not updated annually. Evidence: The plan's last documented update was 3/2018 and was not reviewed March 2019.
Plan of Correction:	The plan was reviewed and the provider signed the form during the inspection.
Standard #:	22VAC40-111-830-C
Description:	Based on provider statement, documentation of monthly evacuation logs was not available. Evidence: The provider recently repainted the day care space and the evacuation log was not re-posted. It could not be found during the inspection.
Plan of Correction:	A new log will be started in May and all logs will be documented moving forward.

Description:	Based on record review, 1 of 2 children's records was incomplete. Evidence: Child #2's two emergency contacts did not have addresses.
