Comments:
An unannounced monitoring inspection was conducted between the hours of 9:35am through 11:15am. There were a total of 6 children present (12 points) between ages 20 months and 3 years old with the Provider. A total of 4 children's files and the provider's file were reviewed. The children were engaged in the following: singing, watching a short educational video, hand washing before and after snack, eating snack which consisted of graham crackers and cheese, and playing outside with the water table. Positive interactions between the provider and children were observed during this inspection. A complete inspection of the programming space, children and staff records, evacuation drills, menu, attendance, and emergency procedures were completed today. No new staff have been hired since the last inspection. The provider advised there was no medication on site today. There was an abundant supply of age appropriate materials and toys and the home was very clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-830-C
Description: Based on record review, documentation of emergency evacuation and shelter-in-place drills was not maintained and available for inspection. Evidence: 1. The provider stated that she has completed monthly fire drills and bi-annual shelter-in-place drills but could not find the drill log.

Plan of Correction: I will locate the drill log.

Standard #: 22VAC40-111-60-B
Description: Based on record review, 3 of 4 children's records were not complete. Evidence: 1. Child #2 and Child #3's records were missing the mother and father's work address and work telephone number. 2. Child #4's record was missing the name, address, and phone number of 1 of 2 emergency contacts required. 3. Child #4's record was also missing an annual review of emergency contact information. Child #4's record was last reviewed by the parent on 5/9/18.

Plan of Correction: Child #2 and Child #3's last day is today. I'm the grandparent of Child #4 so I will get a second emergency contact and I will get the parents to review the file for emergency contacts.
