An unannounced monitoring inspection was conducted on 6/10/19 from 11:00am - 12:15pm. At the time of the inspection there were 11 children in care, ages nine months old to five years old (18 points), with two caregivers present. Records were reviewed for five children and three caregivers/household members. Children were observed playing with outside, playing inside, and eating lunch. There was no medication at the family day home. Emergency plan and emergency supplies were reviewed. Areas of non-compliance are identified on the violation notice and were discussed with the provider during the exit interview.

All newly enrolled children will have a copy of a physical examination in their record within 30 days of enrollment.

Evidence: 1. The record for caregiver/household member #3, contained documentation of TB screening that was dated 1/10/17. 2. The provider reviewed the record for caregiver/household member #3 and confirmed that an updated TB screening had not been received. Plan of Correction: The provider responded: Caregiver/household member #3 will complete an updated TB screening.
