Technical Assistance:
Technical assistance was provided in the following areas of the standards: 22VAC40-191 (Background checks); 22VAC40-111-(2)-60-B (Children's records); 22VAC40-111-(7)-500 (Outdoor play areas and equipment); 22VAC40-111-(11)-800 (Emergency plan); 22VAC40-111-(11)-830 (Emergency drills)

An unannounced monitoring inspection was conducted on 6/10/19 from 11:00am - 12:15pm. At the time of the inspection there were 11 children in care, ages nine months old to five years old (18 points), with two caregivers present. Records were reviewed for five children and three caregivers/household members. Children were observed playing with outside, playing inside, and eating lunch. There was no medication at the family day home. Emergency plan and emergency supplies were reviewed. Areas of non-compliance are identified on the violation notice and were discussed with the provider during the exit interview.

Standard #: 22VAC40-111-100-A
Description: Based on a review of five children's records, it was determined that the provider did not ensure that each child in attendance had a completed physical within one month of attendance. Evidence: 1. The record for child #3 did not contain a physical examination. 2. The provider confirmed that the record for child #3 did not contain a physical examination, and had been enrolled for more than 30 days.

Plan of Correction: The provider responded: The parents of child #3 will be asked to provide a copy of the most recent physical examination. All newly enrolled children will have a copy of a physical examination in their record within 30 days of enrollment.

Standard #: 22VAC40-111-180-A
Description: Based on a review of three caregiver/household member records, it was determined that the facility did not ensure that at least every two years from the date of the first initial screening or testing, caregiver/household member shall obtain and submit the results of a follow-up tuberculosis screening. Evidence: 1. The record for caregiver/household member #3, contained documentation of TB screening that was dated 1/10/17. 2. The provider reviewed the record for caregiver/household member #3 and confirmed that an updated TB screening had not been received.

Plan of Correction: The provider responded: Caregiver/household member #3 will complete an updated TB screening.
