Comments:
An unannounced renewal study was conducted today. There are 12 children present (28 points) with 2 caregivers. 3 adult and 4 children's records were reviewed. See violation notice for missing information. Children were observed resting quietly during the inspection. First aid supplies, emergency supplies, emergency escape drills, shelter-in-place drills, bathroom supplies, and the dog's rabies vaccinations are all in compliance. Description: Based on a review of records, written acknowledgement of the relocation destination for the emergency preparedness plan is not consistent with all records. Evidence: Child #1's record lists a different relocation destination than the emergency preparedness plan.

Plan of Correction: I will review all files to make sure the relocation destination is correct.
