Comments:
An unannounced monitoring inspection was conducted June 12 2019 between 10:45am and 12:15pm. There was one child in the direct care of the provider. The child was observed in a bouncy seat, crawling, and being fed. The home is clean and organized. Areas of non-compliance are listed on the violation notice. Violations:
Standard #: 22VAC40-111-60-B
Description: Based on record review, 1 of 3 children's records reviewed did not contain all of the required information. Evidence: Child #2 record does not have documentation of the child's physician's name and address, the medical insurance information and the policy number.

Plan of Correction: When the parent comes I will ask for the information.

Standard #: 22VAC40-111-70-A
Description: Based on record review, 3 of 3 children's records reviewed did not have documentation that parents of children in care received all the required documents in writing prior to the child's first date of attendance. Evidence: Child #1, #2, and #3 records do not contain documentation signed by parents that they have been notified whether or not the family day home has liability insurance.

Plan of Correction: I will get the liability insurance notification filled out.
