Comments:
A subsidy health and safety inspection was completed on May 29, 2019 from approximately 2:00p.m. to 3:45p.m. to review supplemental health and safety requirements. Two children, preschool age, were in care, with one caregiver for a total of four points. The caregiver and children were observed in the indoor space engaged in activities to include, arising from nap time, snack time and free play. Interviews and interactions were conducted and no medications are given. Five children?s records and two caregiver records were reviewed. The Provider was available for the inspection and was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice.

Standard #: 22VAC40-665-170-B-12
Description: Based on a review of records, the licensee failed to maintain a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction for one of five children. Evidence: 1. Child #3's record, enrolled June 11, 2018, indicated an allergy to peanuts. Child #3's record lacked a written care plan. 2. Caregiver #1 acknowledged the record lacked a plan but advised the child is fed a nut free diet while in care.

Plan of Correction: The provider will obtain the plan from the parent.

Standard #: 22VAC40-665-400-B-7
Description: Based on a review of records, the licensee failed to include a component in the emergency preparedness plan regarding continuity of operations procedures to ensure that essential functions are maintained during an emergency. Evidence: 1. The emergency preparedness plan lacked a component as to how continuity of operations procedures to ensure essential functions are maintain during an emergency will occur. 2. Caregiver #1 acknowledged this was not included in the emergency preparedness plan.

Plan of Correction: The provider will add this procedure to the plan.
