Comments:
The inspector conducted an unannounced mandated inspection from 10:30 until 12:30. There were nine children in care with the provider and assistant present for a total of 23 points. Three violations had been corrected and there were three repeat violations. Children were observed playing with blocks, watching tv, and getting a diaper change. Five children's records were reviewed and two provider records were reviewed. Violations are identified on the violation notice. Please contact your inspector at 276-698-9981 with any questions.

Description: Based on observation, the provider did not follow the most recent, age-appropriate nutritional requirements such as USDA. Evidence: 1. The inspector observed 1/2 sandwich, potato chips and animal crackers being served.

Standard #: 22VAC40-111-920
Description: Based on record review, the center did not list substituted food. Evidence: 1. The inspector observed 1/2 sandwich, chips and animal crackers served at lunch-the menu listed corn dogs, green beans, apple and milk and it was not noted on the menu.

Description: Based on record review, the provider did not obtain physical examinations within 30 days of attendance. Evidence: 1. The inspector reviewed five children's records. One of five (#3) did not have a physical examination and the first day was 9/10/2018.

Plan of Correction: Provider will get e documentation from the parents.

Standard #: 22VAC40-111-30-A
Description: Based on record review and interview, the provider did not comply with the terms of the current license issued by the department. Evidence: 1. The inspector reviewed five children's records. Child #1 was enrolled 5/29/2019 and was younger than 12 weeks old. The provider is licensed for ages 12 weeks-8 years 11 months. 2. The provider verified the infant was enrolled.

Description: Based on record review, the provider did not have updated immunizations of all children. Evidence: 1. The provider reviewed five children's records. Two of five )#2 and #3) did not have updated immunizations.

Plan of Correction: Provider will get the records updated.

Standard #: 22VAC40-111-240-A
Description: Based on observation, the provider did not maintain the outside in a safe condition. Evidence: 1. The inspector observed a landscaping timber that was splintered on the play area and also two metal round posts standing upright presenting a tripping and splinter hazard.

Plan of Correction: The provider will be replacing the timbers with new ones.

Description: The provider failed to keep poisonous supplies stored away from areas accessible to children. Evidence: 1. The inspector observed fingernail polish, hairspray, and Lanacane under the bathroom sink upstairs that children use and the cabinet had a child lock on it but it was broken.

Plan of Correction: The provider will move items so that they are inaccessible to children and/or fix the lock.

Description: Based on interview, the provider did not have a landline. Evidence: 1. The provider stated she does not have a landline.

Plan of Correction: The provider stated she has submitted the paperwork allowable variance to the inspector.

Standard #: 22VAC40-111-460-C
Description: Based on observation and interview, the provider did not have a dog vaccinated for rabies. Evidence: 1. The inspector asked for the documentation; during the inspection the provider called the vets office and was told the rabies vaccine had expired as of May 10, 2019.

Plan of Correction: The provider has an appointment for the pet 06/22/2019.

Description: Based on observation, the cribs did not have tight-fitting bottom covers. Evidence: 1. The inspector observed three cribs and two of the three did not have tight fitting covers One infant was asleep on the end of the crib sheet that was the width that had exposed foam due to the loose fitting cover.

Plan of Correction: Provider will make or purchase tight-fitting sheets

Standard #: 22VAC40-111-680-A
Description: Based on observation, the provider did not wash hands after changing a diaper and before preparing food. Evidence: 1. The inspector observed the provider during the process not washing her hands.

Plan of Correction: Provider will wash her hands in the future after diapering a child and before preparing a bottle/food.

Standard #: 22VAC40-111-690-D
Description: Based on observation, the provider failed to meet the requirement for diapering surface. Evidence: 1. The inspector observed one diaper being changed on the floor/carpert/and the other diaper being changed on couch.

Plan of Correction: The provider will change diapers on an approved diapering surface/one is on order.

Description: Based on observation, the provider did not have posted special order dated April 26,2019.

Plan of Correction: Provider will post it today.
