06/06/2019 Arrived 10:15am Departed 1:00pm An unannounced monitoring inspection was conducted today. Twelve children were in care for a total of 20 points with one caregiver. The children were playing outdoors, playing with toys. Eighteen children's files and two staff files were reviewed. **Not all areas of the key health and safety standards could be reviewed because the caregiver was unable to gather the items while supervising the children outdoors. Standard #: 22VAC40-111-570-A
Description: Based on observation the provider failed to insure that the points requirement (16 per caregiver) was not exceeded. Evidence: 1. Upon arrival, there were 11 children in care for a total of 21 points with one caregiver. 2. At 11:30am child #15 left, leaving 10 children and a total of 19 points with one caregiver. 3. At 11:40am child #19 & 20 arrived. Twelve children are present for a total of 20 points with one caregiver.

Plan of Correction: The helper had an emergency and left. He came back just as soon as he could. I'm going to talk with another lady about being a substitute for me when something like that happens.
