Discussed center licensing process and director requirements, application renewal process, and required assistant caregiver documentation

Comments:
An unannounced mandated monitoring inspection was conducted today from 10:30 am to 12:30pm. There were 5 children present (8 points) and 1 caregiver during this inspection. Children were observed walking with a walking toy, sliding on a slide, playing with puzzles and legos. Reviewed 3 children's files, 1 caregiver file, 2 household member files, emergency evacuation log and supplies, attendance, menus, cribs, and physical plant. Violations:
Standard #: 22VAC40-111-100-A
Description: Based on record review, provider shall did not obtain documentation of a physical examination by or under the direction of a physician within 30 days after the first day of attendance. Evidence: Child #1, Child#2 and Child#3 did not have documentation of a physical from a physician within 30 days of their first day of attendance. (Start dates Feb 2016; July 2017; April 2019)

Plan of Correction: I will ask the parents for copies of the physicals. Child#1 last day is tomorrow.

Standard #: 22VAC40-111-60-A
Description: Based on record review, children's record were incomplete. Evidence: 1. Child#2 and Child#3 did not have proof of the child's age and identity. 2. Child #1 and Child#2 did not have documentation of the annual review of the child's emergency contact information

Plan of Correction: I will get the information from the paprents

Standard #: 22VAC40-111-330-A
Description: Based on observation and interview with provider, a landline telephone is not available or accessible during the home's operating hours. Evidence: The home does not have a landline phone. Interview with Provider revelaed she only has a cell phone available.

Plan of Correction: I will get one.
