Licensing Inspector arrived at 10:00 and departed at 11:15 AM. At the time of the inspection, there were 10 children (24 points) in care with the provider and an assistant. Children were observed participating in free play activities. All children's records were reviewed and other documents were reviewed.

Violations:

Standard #:	22VAC40-111-750-A
Description:	Based on observation, it was determined that the provider did not ensure that there was parental authorization available for diaper ointment. Evidence: The provider had several different diaper ointments and powders under her changing table however, there were no parental authorizations.
Plan of Correction:	Provider will have the parents complete authorizations.
Standard #:	22VAC40-111-60-A
Description:	Based on review of 14 children's records, it was determined that the provider did not ensure that all elements were present in each child's record. Evidence: The records for child #1 and child #5, who were in care at the time of the inspection, were missing emergency contact information.
Plan of Correction:	Provider will obtain emergency information and place in the records.
Standard #:	22VAC40-111-90-A
Description:	Based on review of 14 children's records, it was determined that the provider did not ensure that before each child attends the family day home, they had immunizations on file. Evidence: The record for child #3, who was in care at the time of the inspection, did not have immunizations on file.
Plan of Correction:	Provider will obtain immunizations and place them in the record.
Standard #:	22VAC40-111-690-D
Description:	Based on interview it was determined that the provider did not ensure that the diapering surface is cleaned and sanitized after use. Evidence: The provider stated that the diaper changing surface was cleaned and sanitized using Clorox wipes rather than spraying the surface and allowing it to air dry.
Plan of Correction:	Provider will begin using a spray and allowing it to air dry.
