Comments:

An unannounced renewal inspection was conducted today from approximately 10am-12pm. There were no children in care. A sample of 3 children?s records and 1 caregiver's record were reviewed. The provider is in the process of hiring one caregiver and one volunteer. Zero new caregivers have been hired since the last inspection. Areas of non-compliance are identified in this report. Standard #:	22VAC40-111-100-A
Description:	Based on record review, the provider did not obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance. Evidence: Children A and B did not have documentation of a physical exam on file.
Plan of Correction:	I will get them from the parents as soon as possible.
Standard #:	22VAC40-111-240-A
Description:	Based on observation, furnishings of the home were not maintained in an operable condition. Evidence: The door to the entrance of the room where children watch television had a broken top hinge.
Plan of Correction:	I will fix it as soon as possible
Standard #:	22VAC40-111-320-G
Description:	Based on interview, a wood burning fireplace was not inspected annually. Evidence: Caregiver A stated that the fireplace was wood burning and it had not been inspected.
Plan of Correction:	I will apply for an allowable variance.
Standard #:	63.2-1720.1-B-2
Description:	Based on record review, caregivers working directly with children did not have complete fingerprinting results. Evidence: Caregiver A did not have complete fingerprinting results on file.
Plan of Correction:	I will request a duplicate of the findings.
