Comments:
A Subsidy Health and Safety inspection was conducted on 06/10/2019 to review supplemental health and safety requirements. An unannounced renewal inspection was conducted today from approximately 10am-12pm. There were no children in care. A sample of 3 children?s records and 1 caregiver's record were reviewed. The provider is in the process of hiring one caregiver and one volunteer. Zero new caregivers have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. If you have any questions, contact Kimberly.Sawyer@dss.virginia.gov. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Description: Based on record review, the vender did not maintain a record of daily attendance that documents the arrival and departure of each child. Evidence: The attendance kept did not document the departure of each child.

Description: Based on record review, the emergency preparedness plan was incomplete. Evidence: The emergency preparedness plan was missing lockdown procedures.

Description: Based on record review, the evacuation and relocation procedures were incomplete. Evidence: The evacuation and relocation procedures were missing plans to reunite parents with children after the emergency.
