The provider has stated that they have not had any children utilizing the state subsidy for a very long time and that they have tried to terminate their contract. The provider no longer wishes to be a subsidy provider and will be terminating their vendor agreement.
- Comments:

A Subsidy Health and Safety monitoring inspection was conducted on 6/27/2019 to review supplemental health and safety requirements. This inspection began at 3:10pm and ended at 4:00pm. There were 2 caregivers with 11 children at the time of the visit. At the start of the inspection, children were observed getting up from nap, pottying, playing freely and watching TV. Emergency preparedness procedures, fire drill log were reviewed. The physical plant and playground were inspected. There were no medications on site at this time. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. No violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Thank you for your cooperation today.
