22VAC40-665 Subsidy Program Vendor Requirements for Family Day Homes

Comments:
A Subsidy Health and Safety inspection was conducted on 6/27/19 to review supplemental health and safety requirements. Upon arrival to the family day home the older children were preparing breakfast. The other children played with the available toys. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Inspection began: 9:30 am ended 12:00 pm

Violations:
Standard #: 22VAC40-665-160-D
Description: Based on review of records the vendor failed to have a hard copy daily attendance record that documents arrival and departure. Evidence: 1. The attendance record does not document arrival and departure of the children.

Plan of Correction: The vendor will add this information to the form that is being used.

Standard #: 22VAC40-665-170-B-12
Description: Based on review of records the vendor failed to have a written allergy plan for children in care. Evidence: 1. Child # 4 & 6 have an allergy to a food but, there are no written allergy plans on file.

Plan of Correction: The vendor will discuss this with the parent and will have an allergy care plan completed.
