Comments:

An unannounced inspection was conducted today, 6/27/19. Upon arrival to the family day home the children were playing with one another and the older children were preparing breakfast. The children then ate breakfast and following breakfast they played video games and sang songs. A violation notice was completed during the inspection. Standard #:	22VAC40-111-870-G
Description:	Based on observation the provider failed to clean the tables appropriately. Evidence: 1. The tables were cleaned with a Clorox wipe prior to eating meals.
Plan of Correction:	The provider will ensure that tables and high chair trays are cleaned with soap and water.
Standard #:	22VAC40-111-900-B
Description:	Based on observation the provider failed to ensure that all children do not walk around while drinking beverages. Evidence: 1. During the inspection a child went to the refrigerator and retrieved a Coke and then walked around drinking this Coke.
Plan of Correction:	The providere will ensure that all children sit when drinking or eating.

Description:	Based on observation the provider does not have a menu posted. Evidence: 1. The provider does not have a menu posted.
Plan of Correction:	The provider will post a menu of the meals or snacks provided by the provider.

Description:	Based on review of children's records the provider failed to ensure that all children in care have an up-to-date record. Evidence: 1. Child #5 does not have a record. The provider reported that said child has been in care since June 1, 2019.
Plan of Correction:	The provider will talk with the parents and have a record set up.

Description:	Based on review of records the provider failed to have the appropriate forms completed regarding immunizations. Evidence: 1. Children's records # 1 & 2 has a statement that the children do not have immunizations due to religious reasons. This information is not in the required format. (Virginia Department of Health form)
Plan of Correction:	The provider will ensure that the parents fill out the appropriate form.

Description:	Based on observation the provider failed to ensure that all children wash their hands with liquid soap and warm running water. Evidence: 1. The children wash their hands prior to eating breakfast with wipes.
