The licensing inspector conducted an unannounced monitoring inspection Wednesday, June 26, 2019 from approximately 9:40 AM until 11:00 AM to check if violations from a previous inspection were corrected and monitor compliance with Family Day Home standards. There were 9 non-resident children and one resident child present during the inspection. Two caregivers were supervising the children for a total of 23 points. Children were observed watching children?s programs on the T.V., writing and coloring and singing songs. During the inspection, one child?s record was reviewed. The two caregivers? records were reviewed during the February 2019 inspection and no changes have been made. Standard #: 22VAC40-111-100-A Description: Based on review of one child's record, the provider did not ensure documentation of a physical examination by or under the direction of a physician was obtained prior to a child's attendance or within 30 days after the first day of attendance. Evidence: The record for Child #1 (start date 5/15/18) did not contain documentation of a physical exam. Plan of Correction: I will ask the parent for the paperwork.
