54.1-3408 Provider must be MAT certified to administer prescription medication.

Technical Assistance:
Discussed the importance of improving the overall level of compliance, particularly in the area of record keeping. Reminded the provider that all cribs shall be spaced at least 12 inches apart when occupied.

Comments:
An unannounced monitoring inspection was conducted today. The provider and two assistant caregivers were caring for 12 children, totaling 31 points. Children's records, caregiver records, the physical plant, children's programming, children's nutrition, and emergency preparedness items were inspected today. The children were observed playing with toys, watching a children's television program, and preparing for lunch. The children were served macaroni and cheese, broccoli, grape slices, and milk for lunch today. This inspection was completed between approximately 10:30am and 12:10pm. Violations:
Standard #: 22VAC40-111-750-A
Description: Based on observations, it was determined that one child's topical ointment product was not labeled as required. Evidence: One child's diaper ointment was not labeled with her name.

Plan of Correction: It will be labeled today.

Standard #: 22VAC40-111-920
Description: Based on provider interviews and observations, it was determined that the menu for the current one-week period has not been completed. Evidence: The provider has not created a menu for the current one-week period.

Plan of Correction: I am only open 2 days this week. I will create a menu when I get back from vacation.

Standard #: 22VAC40-111-100-A
Description: Based on a review of children's health records, it was determined that two records were not complete. Evidence: Child records A and E do not include documentation of the child's physical exam report. Each child has been enrolled for longer than 30 days.

Plan of Correction: I gave the form to each parent. I will follow up to make sure they return it.

Standard #: 22VAC40-111-110-A
Description: Based on a review of children's immunization records, it was determined that they have not been completed on the correct Virginia Department of Health form. Evidence: The children's shot records do not include the physician's signature on the statement indicating that each child has been adequately immunized for his/her age. This statement is standard wording on the Virginia Department of Health form.

Plan of Correction: I will ask each parent to bring the VA form completed, with this statement signed by the doctor.

Standard #: 22VAC40-111-120-B
Description: Based on a review of caregiver records, it was determined that two records are not complete. Evidence: Caregiver record A does not include: two reference checks; verification of the person's age. Caregiver record B does not include verification of the person's age.

Plan of Correction: I will make sure both files are complete.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, it was determined that one record was not complete. Evidence: Child record B does not include the information for a second emergency contact person.

Plan of Correction: I will ask the parent for the information.

Standard #: 22VAC40-111-200-A
Description: Based on a review of caregiver training documents, it was determined that the provider did not conduct Orientation Training within the employee's first week of hire. Evidence: The provider has not conducted Orientation Training for caregiver B, who began employment in 9/18.

Plan of Correction: I will do the training again, and document it.

Standard #: 22VAC40-111-240-A
Description: Based on observations, it was determined that one are of the home has not been maintained as required. Evidence: In the basement child care area, one section of the window blinds has broken. Pieces of the blinds have broken off and some pieces are still hanging. The damaged section of the blinds is at the children's level.

Plan of Correction: When I get back from vacation I was already planning to remove the blinds and install curtains instead.
