Technical Assistance:
The provider recently installed a new piece of play equipment in the backyard play area. This play structure is indicated to be appropriate for children age 3 - 10 years only. The provider has already spread some mulch around this play structure, but the rest of the mulch shall be distributed as soon as possible. There is also a stack of mulch bags on the concrete area in the backyard. These bags must be moved away from the concrete, as they are currently stacked and in an area where the children could climb them. Reviewed the administrative requirements in order to have a long term adult visitor in the home, as well as the requirements for hiring an additional assistant caregiver.

Comments:
An unannounced full renewal inspection was conducted today. The provider and one assistant caregiver were caring for 11 children, totaling 28 points. Administrative records, emergency preparedness items, children's records, background checks, caregiver records and training, child:caregiver interactions, and children's nutrition were inspected today. The children were observed making flag paintings, playing outside, singing/dancing to music, listening to books, and playing with toys. The children were to be served beef with lentils and rice, sliced grapes, and milk for lunch today. This inspection was completed between approximately 10:15am and 12:15pm. Violations:
Standard #: 22VAC40-111-960-C
Description: Based on observations, it was determined that two infants under the age 12 months were seated in high chairs without the safety straps fastened. Evidence: Upon arrival, the Inspector observed two infants (both age 8 months) seated in high chairs without the waist safety strap fastened.

Plan of Correction: The safety buckles were fastened immediately during the inspection.

Standard #: 22VAC40-111-960-E
Description: Based on observations, it was determined that infant bottles were not labeled as required. Evidence: Two infant bottles were not labeled with each child's name. There were a total of 3 infants in care.

Plan of Correction: The bottles will always be labeled.

Standard #: 22VAC40-111-80-C
Description: Based on a review of children's records, it was determined that the provider did not document viewing one child's original proof of identification. Evidence: Child record E does not include documentation showing that the provider has viewed the child's original proof of identification.

Plan of Correction: I did see her birth certificate and there is a copy in the file. I updated the documentation during the inspection.

Standard #: 22VAC40-111-170-A
Description: Based on a review of administrative documents, it was determined that one new adult household member has not obtained a TB health screening. Evidence: The provider currently has one adult long-term household visitor living in the home. This person has already interacted with the children. The provider has not asked this person to obtain a TB health screening.

Plan of Correction: She is out of town for a couple of days. I will make sure she gets her TB test done when she returns.

Standard #: 22VAC40-111-570-A
Description: Based on observations, it was determined that the provider was alone caring for more than 16 points of children. Evidence: Upon arrival, the provider was alone caring for a total of 28 points of children. The provider was alone with 28 points of children for approximately 30 minutes.

Plan of Correction: My assistant stepped out to pick up our own children from the pool. Normally we have another person pick up our children, but that person is not available today. Moving forward I will make sure no caregiver is ever alone with more than 16 points of children.
