Technical Assistance:
Discussed that children may not play on the trampoline in the back yard.

Comments:
An unannounced monitoring inspection was conducted today from 7:30am-9:30am. There were 4 children (8 points) directly supervised by the provider and two assistants. The child care area, 2 staff records, 4 children?s records, 1 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely watching a children's music program. Hand washing procedures were also observed. There was an assortment of age appropriate materials for children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-800-B
Description: Based on record review, the provider did not update the emergency plan at least annually and update the plan as needed. Evidence: The most recent documented update of the emergency plan was dated 3/20/2019.

Plan of Correction: I will update the plan today.

Standard #: 22VAC40-111-100-A
Description: Based on record review, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence: Child 3's record does not contain documentation of a physical examination by or under the direction of a physician. The child was enrolled in the program on 5/27/2019.

Plan of Correction: I will ask the parents to provide a physical.

Standard #: 22VAC40-111-90-C
Description: Based on record review, one child's record did not contain documentation of additional immunizations for a child who is not exempt from the immunization requirements. Evidence: Child 3 (Date of birth 5/27/2017) has an immunization record which documents the most recent immunization was received on 6/22/2018.

Plan of Correction: I will ask the parent to provide an updated record.

Standard #: 22VAC40-111-240-A
Description: Based on observation, not all equipment in the family day home was maintained in a clean and safe manner. Evidence: Two of the molded plastic swings in the backyard are corroded which may weaken the plastic making them unsafe for use.

Plan of Correction: We are not currently using the swings, but we will remove or replace them.

Standard #: 22VAC40-111-320-B
Description: Based on observation, child resistant protective covers were not installed on all outlets accessible to children. Evidence: Children were observed upstairs in the living area of the family day home where outlets in the hallway, living room and kitchen were accessible and did not have protective covers.

Plan of Correction: I will install protective covering upstairs.

Standard #: 63.2-1813
Description: Based on record review and provider statement, a custodial parent was not admitted to the family day home while their child was in care. Evidence: 1. Child 1's record contains the name of both biological parents. Under the section "Persons authorized to Pick Up Child" the mother has written "Permission is needed if anybody but mother trys [sic] to pick up". 2. The provider stated that "several months ago" the father listed attempted to pick up the child and was denied because the provider had not obtained prior permission from the father. 3. A custody agreement was not present in the child's record.

Plan of Correction: I will ask the parent to provide custody paperwork and inform her of the rights of parents.
