Technical Assistance:
The provider shall submit documentation of CPS search request and the findings results to the Inspector. The license cannot be renewed until these documents are submitted.

Comments:
An unannounced full renewal inspection was conducted today. The provider and two assistant caregivers were caring for 7 children, totaling 20 points. Children's records, administrative documents, background checks, caregiver/adult records, emergency preparedness items, the indoor/outdoor physical plant, children's medications, and caregiver interactions were inspected today. The children were observed playing with toys, eating lunch, and interacting with each caregiver. The children were served fish sticks, mixed vegetables, watermelon, and milk for lunch today. This inspection was completed between approximately 10:45am and 12:30pm. Thank you for meeting with me today. Violations:
Standard #: 22VAC40-111-700-C
Description: Based on a review of children's medications, it was determined that one child's nonprescription medication was not sent home within 14 days of the expiration of the authorization. Evidence: One child's medication authorization expired on 11/26/18. The medication was still on site as of the date of this inspection 7/3/19.

Plan of Correction: I will send the medication home today.

Standard #: 22VAC40-111-750-A
Description: Based on observations, it was determined that two topical skin products were not labeled as required. Evidence: Two children's diaper ointment products were not labeled with the child's name.

Plan of Correction: I will label them today.

Standard #: 22VAC40-111-800-B
Description: Based on a review of administrative documents, it was determined that the provider did not conduct an annual review of the emergency preparedness plan. Evidence: The provider most recently reviewed the emergency plan on 2/10/18.

Plan of Correction: I will review the plan with my assistant today.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, it was determined that three records were not complete. Evidence: 1- Child record B does not include the information for a second emergency contact person. 2- Child record A and record D do not include documentation showing that each parent has reviewed their child's file within the past year.

Plan of Correction: The parent listed the same person twice. I will ask the parent for another contact person.

Standard #: 22VAC40-111-70-A
Description: Based on a review of children's records, it was determined that one parent has not been given all required written information. Evidence: The provider has not given her writtenemergency plan information to the parent of child B.

Plan of Correction: I will ask the parent to sign my emergency plan form.

Standard #: 22VAC40-111-540-A
Description: Based on observations, it was determined that two children were not seated in the activity table with the safety strap buckled. Evidence: The provider observed two children using the activity table. This table is equipped with bucket seats that have safety straps. These children were not seated with the safety strap buckled. The Inspector observed one of the children climbing on top of the table.

Plan of Correction: The children were seated with the safety strap immediately during the inspection. I will make sure the safety strap is always used.

Standard #: 22VAC40-191-40-D-5-a
Description: Based on a review of background checks, it was determined that the provider has not requested one required background search for a household member. Evidence: Household member D came of age to require a search of the VA Central Registry on 3/21/19. The provider has not requested this background search.

Plan of Correction: We will complete the CPS search request form this weekend and send it in on Monday. I will also send a copy of the request to my Inspector within 10 days, as required.
