An unannounced renewal study was conducted today. There are 4 children present ( 7 points) with 1 caregiver. 5 adult and 4 children's records were reviewed. See Violation notice for missing information. Children were observed during diapering, hand washing, plying with toys and preparing for lunch. Emergency evacuation drills, emergency supplies, menu, and bathroom supplies are all in compliance. Evidence: Missing information includes but may not be limited to; *Second emergency contact complete address. * Health insurance policy number (Child A) Plan of Correction: I will ask the parent to provide the missing information.
Standard #: 22VAC40-111-70-A Description: Based on a review of records, one child's record does not include documentation of all required policies and procedures. Evidence: Missing policies include but are not limited to: Holidays, payment schedule, schedule of the day, DSS website address. Plan of Correction: I will have the form completed today.
Standard #: 22VAC40-191-40-D-5-a Description: Based on a review if records, one household member's (HHM) records is incomplete. Evidence: HHM #1's record does not include a completed Central Registry Search (CPS) within 30 days of turning 14 years old. (Date of Birth 1/27/05). Form given to the provider. Plan of Correction: I will submit the form as soon as possible. I will send you the results once I get them.
