A Subsidy Health and Safety inspection was conducted on 7/2/19 to review supplemental health and safety requirements. An unannounced monitoring inspection was completed on 7/2/19. There were 11 children in care from 22 months through five years with the provider and two assistants supervising. The point total was 20. One newly enrolled child's record was reviewed and four other children's records were reviewed for annual updates. The provider's record and one assistant's record were reviewed. The provider does not administer medications. The inspector arrived at the provider's home at 1:30 pm and departed at 4:00 pm. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. No violations of the supplemental subsidy health and safety requirements were found.
A compliance history is in no way a rating for a facility.
The online compliance history includes only information after July 1, 2003. In addition, the online compliance history includes information regarding adverse actions that may be the subject of a pending appeal. An adverse action is not final until a provider has exhausted or waived all due process rights. For compliance history prior to July 1, 2003, or information regarding the status of pending adverse actions, please contact the Licensing Inspector listed in the facility's information.
