54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced renewal inspection was conducted today between the hours of 9:15 am through 10:20. There were a total of 6 children (16 points) with 2 caregivers. A total of 3 children's files and 4 staff/household member's files were reviewed. The children were engaged in the following: breakfast, hand washing, toy play, diapering and manipulatives. Standard #: 22VAC40-111-60-A Description: Based on a review of children records, the provider has not maintained a complete record for each child. Evidence: Child #1, first day of attendance 6/24/19 does not have the child record that contains information about the child, parent contact information, emergency contacts, proof or identity, and authorization for emergency care. Plan of Correction: There is a record on file for the child's brother with all the parent contact information. I will ask the parent for the forms.
Standard #: 22VAC40-111-70-A Description: Based on children record review, before the child's first day of attendance, parents have not been provided in writing the homes policies. Evidence: There is no documentation from the parents for child #1 about the homes policies. Plan of Correction: I will get the signed forms from the parents.
