Technical Assistance:
Reminders: - Renewal application must be sent to the Fairfax licensing office prior to the license expiration date - Caregiver/household member TB test/screenings will need to be completed again in the next few weeks.

Comments:
An unannounced renewal inspection was completed today. The physical plant, 3 caregiver/household member files, 2 children's files, caregiver training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected between 12:30-1:40. There were 2 children, totaling 7 points, under the supervision of 2 caregivers. During the inspection, children were observed resting at nap time. Today, the children were served grilled cheese sandwiches (on whole wheat bread), mangos, carrots and milk. The Provider is not certified in MAT (medication administration training) and there are no children with any medication needs at this time. Standard #: 22VAC40-111-830-B
Description: Based on interview. the Provider failed to practice a minimum of 2 Shelter-in-place (SIP) drills each year. Evidence 1. There was no documentation of having completed 2 SIP drills in 2018.

Plan of Correction: One drill was recently completed. This will be done each year.

Standard #: 22VAC40-111-60-B
Description: Based on file review, 1 out of 2 children's records does not have all required information. Evidence 1. Child #1 does not have the name, address and telephone number of both custodial parent's place of employment or the address of both emergency contacts in case the parents cannot be reached.

Plan of Correction: The Provider will ask the parents for this.

Standard #: 22VAC40-111-70-A
Description: Based on file review, 1 out of 2 children's records does not have all required information provided in writing prior to the child's first day of attendance. Evidence 1. Child #1 does not have a signed written policy for the administration of medications or the "Provisions Of the Emergency Preparedness and Response Plan" on file.

Plan of Correction: The Provider will give this to the parents to review and sign.
