Technical Assistance:
Technical assistance for provided for the following: MAT training, annual 16 hours training, infant sleep and feeding positions and techniques.

Comments:
An unannounced renewal inspection was conducted from 11:15am to 12:45pm. There were 8 children (24 points) in care of 3 caregivers. The children were observed eating lunch brought from home. The children were also observed washing hands, being put to sleep for nap time and getting bottle fed. Emergency drill records, first aid kit and other documents were reviewed. 4 children's records, 3 household member records and 3 caregiver records were reviewed. A complete walkthrough of the family day home was conducted. Areas of non compliance are listed in the violation notice. Violations:
Standard #: 22VAC40-111-830-A
Description: Based on record review, the provider did not practice evacuation drills each month. Evidence: Caregiver #1 stated that she did not practice an evacuation drill for the month of June 2019.

Plan of Correction: I will do a make up drill for the month of June in July.

Standard #: 22VAC40-111-60-B
Description: Based on record review, children's records were missing information. Evidence: 1. Child # 1's record was missing an address for 1 of 2 emergency contacts. Child # 1's record was also missing medical insurance policy number. 2. Child # 2 and Child # 3's record was missing information for persons authorized to pick up.

Plan of Correction: I will get the information from the parents.

Standard #: 22VAC40-111-180-A
Description: Based on records review, the provider did not obtain subsequent tuberculosis (TB) screening reports every two years, for all adult caregivers and household members. Evidence: 1. Household Member # 1's most recent TB screening expired on 5/4/19. 2. Caregiver # 1's most recent TB screening expired on 4/27/19.

Plan of Correction: I will get updated TB screenings.
