An unannounced monitoring inspection was conducted today between the hours of 9:15 am through 10:30. There were a total of 3 children (7 points) with 1 caregiver. A total of 3 children's files and 3 staff/household member's files were reviewed. The children were engaged in the following: toy play, cars and colors. Standard #: 22VAC40-111-60-B Description: Based on a review of children records, each child's record is not complete. Evidence: Child #1 and #2, first day of attendance 6/17/19 does not have documentation of the medical insurance policy number. Plan of Correction: I will ask the parent for this information.
Standard #: 22VAC40-111-90-A Description: Based on a review of children records, before a child may attend the provider has not obtained documentation of children's immunization record. Evidence: There is no documentation of immunization record for child #1 and child #2, both first day of attendance 6/17/19. Plan of Correction: I will ask the parent to provide the immunization record.
Standard #: 22VAC40-111-180-A Description: Based on a review of caregiver and household records, the provider has not maintained a current report of tuberculosis (TB)screening every two years from the date of the first screening. Evidence: Caregiver #1 does not have a current TB screening, last screening dated 7/1/17. Household #2 does not have a current TB screening, last screening dated 7/1/17. Plan of Correction: TB screening scheduled in another 2 weeks.
