Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and the renewal process. Further discussion was had on the topic of the backyard with regards to the steps (3 set of risers, and the 8 step of risers); and the new playground equipment with regards to fall zones. The platform of this new equipment was measured at just under 4 feet in height.

Comments:
The following inspection took place between the hours of 9:30 am through 11:30 am. Today, I met with the Provider on site at the Family Day Home (FDH) located in Aldie, Virginia. At the start of this unannounced renewal inspection there were a total of 6 children (13 points) with 2 caregivers. A total of 6 children?s files, 2 staff files, and 2 household member?s files were reviewed. The children were engaged in the following: toy play, morning snack, hand washing, bathroom break, learning, music, fun with play-doh, imaginative play, getting ready for lunch, and other organized activities. Violations:
Standard #: 22VAC40-111-210-A
Description: Based on review and interview, 2 of 2 staff files reviewed indicated that a minimum of 16 clock hours of training annually in areas relevant to their job responsibilities were not obtained. Evidence - 1. On the date of inspection (07/09/2019), Staff #1 had a total of 8 hours of annual training. 2. Staff #2 had a total of 0 hours of annual training. 3. Staff #1 stated (during an interview conducted on the date of inspection) that a training that had been scheduled was canceled at the last minute. Both Staff #1 and Staff #2 were signed up to take this course.

Plan of Correction: 16 hours of annual training shall be completed for all applicable staff for the upcoming annual training year.

Standard #: 22VAC40-111-430-C
Description: Based on observation, children over the age of two had access to stairs, outside, with three or more risers that do not have a protective barrier or guardrails on each side. Evidence - 1. On the date of inspection (07/09/2019), the back steps leading to the main (back) entrance to the Family Day Home (FDH) consisting of a total of three (3) risers. There are also a set of stone stairs leading from the grate next to the parking area, along the side yard, down into the backyard consisting of a total of eight (8) separate risers. 2. This gate and entrance is used by the parents and children when entering the FDH, and exiting into the back yard for play. 3. On the date of inspection there were a total of 5 children over the age of two years. 4. This set of 3 risers contains handrails on one side. There are no handrails, or barriers for the set 8 risers (stone stairs). 5. This same standard was cited during an inspection conducted on 04/09/2019.

Plan of Correction: A secured barrier and/or hand railing shall be installed. If needed a new route through the house may be directed.

Standard #: 22VAC40-111-500-C
Description: Based on observation, stationary outdoor playground equipment has been placed at least six feet from an obstacle. Evidence - 1. On the date of inspection (07/09/2019), the newly installed outdoor play equipment located in the backyard was observed. 2. The arch of the left swing, as well as its fall zone, is within a six foot radius (in regards to nearby obstacles, other equipment, or perimeter) of a stone sidewalk.

Plan of Correction: The equipment shall be moved away from this stone sidewalk. The sidewalk may also be taken up in order to add space to fit this equipment into the installed area. The side bench to the left may also be removed along with the right side swing.
