Technical Assistance:
Discussed using the VA state model forms for children and staff records to ensure that all required information is documented.

Comments:
Today an unannounced Renewal inspection took place between 9:30am and 12:00pm. There were 8 children present with the provider and 1 assistant for a total of 25 points within the supervision guidelines. The children were observed playing freely with books and various toys and eating lunch. A complete inspection of the physical plant, children and staff records, fire drill log, attendance and emergency procedures were observed during this inspection. Thank you for your cooperation. Violations:
Standard #: 22VAC40-111-960-E
Description: Based on observation, bottles were not labeled with today's date. Evidence: 3 out of 5 bottles observed in the provider's refrigerator did not contain today's date.

Plan of Correction: I have asked the parents to do it. I have tape to do it myself for when the parents forget.

Standard #: 22VAC40-111-60-B
Description: Based on record review, children's records did not contain all required information. Evidence: Child #2 file does not contain a second emergency contact or the families insurance information. Child #6 file does not contain documentation of an annual review of information. Child #7 file does not contain the families insurance information or an annual review of information. Child #9 file does not contain the families insurance information or the information for parents form. Child #10 file does not contain documentation that the child's identity has been verified or the families insurance information.

Plan of Correction: I wrote everything down so as soon as the parents pick up today I'll be able to request the information.

Standard #: 22VAC40-111-170-A
Description: Based on record review, the provider has not obtained a tuberculosis (TB) screening for each household member. Evidence: Household member #3 file does not contain TB screening results.

Plan of Correction: I have the immunization record from when he received the shot. He went back for the results but he needed to take them to the hospital where he works. I called the nurse today and they told me that it was negative, but I will have to log into the online portal to print out the results.

Standard #: 22VAC40-111-280
Description: Based on observation, potentially poisonous substances were not stored in areas inaccessible to children. Evidence: There are two spray bottles of sanitizer spray stored on the glass shelf above the sink in the children's bathroom.

Plan of Correction: I'll move them into another cabinet where the children can't reach. This was corrected during the inspection.

Standard #: 22VAC40-111-340-C
Description: Based on observation, the bathroom did not contain all required supplies. Evidence: There is no paper towels available in the children's bathroom.

Plan of Correction: I have them in my car. I will get them in there.
