Technical Assistance:
Discussed the importance of tight-fitting sheets when cribs are used. Please review your topical ointments periodically to ensure ointments are not maintained after their expiration date.

Comments:
An unannounced Renewal inspection was conducted this date. A total of 7 children (21pts) with Provider and 1 Assistant present during the inspection. When the Inspector arrived, five of the seven children were preparing to color a traced fish on construction paper. Two infants were napping under the supervision of staff in the crib room. Once the infants awoke, they joined the group of children and were placed in areas of the programming space to ensure their safety from the older children. Hand washing procedures observed prior to lunch. Lunch today was chicken, apple sauce and corn. Infants were observed being held and fed. All 7 children's records, all staff and household records were reviewed today. No medications on site as this provider does not administer medication. Standard #: 22VAC40-111-80-A
Description: Based on child record review, proof of identity for one child was not verified. Evidence: Verification of Child #4's age and identity was not obtained.

Plan of Correction: I will verify this child's birth certificate and document the verification.

Standard #: 22VAC40-111-90-C
Description: Based on review of children's records, additional immunizations were not obtained. Evidence: Child #4 is under the age of 2 years. Additional immunizations were not obtained every six months. The last documented immunizations found were dated 8/2018.

Plan of Correction: I will request the updated immunizations from the parent today.

Standard #: 22VAC40-111-240-A
Description: Based on observation of the crib room, one area of furnishing was not maintained in a safe condition. Evidence: A crib located in the crib room had a mattress which was cracked in multiple areas.

Plan of Correction: We will dispose of this mattress and obtain a new one.

Standard #: 22VAC40-111-530-B
Description: Based on observation of the crib room, two cribs did not have a tight-fitting bottom cover Evidence: The two smaller cribs located in the crib room had crib sheets that were not tight-fitting.

Plan of Correction: I will replace these sheets with smaller tight fitting sheets.
