Comments:
An unannounced Renewal inspection was conducted this date. A total of 11 children (23pts) with Provider and 2 assistants present. When the inspector arrived this morning, the children were seated and coloring and tracing the letter "A". A nice transition was observed from morning table top activities to a large motor activity where the children were singing and dancing and swimming like sharks with staff. The older children continued to work on worksheets with encouragement and guidance provided by staff. Inspection of the crib area was made. A total of 3 cribs in use at this time. No medications on site at this time. The provider's home was inspected and found clean and organized. Hand washing observed of all children prior to lunch. Lunch today was chicken, rice, peas and grapes. Staff were spaced apart in groups of children to assist with feeding. Sanitizing of tables, high chairs and trays was observed directly after lunch. Circle time was provided after lunch where staff read a book just before nap time. A total of 6 children's records, all staff and household records were reviewed. Previous violations from the last inspection in April were reviewed and corrective actions were verified. This inspection began at approximately 10am and ended at 12:15pm. Standard #: 22VAC40-111-800-A
Description: Based on review of documentation and operational records, an emergency preparedness plan was not maintained. Evidence: Provider was unable to find the emergency preparedness plan for the family day home.

Plan of Correction: The plan has been written and maintained for years. A new plan will be completed if the existing plan can not be located.

Standard #: 22VAC40-111-180-A
Description: Based on staff and household record review, subsequent TB screenings were not obtained. Evidence: Staff #2 and Household #1's last TB screening was in June 2017. TB screenings are required every two years.

Plan of Correction: We will have the TB screenings completed.

Standard #: 22VAC40-111-690-D
Description: Based on observation of the programming area, the diapering surface was no longer absorbent and surfaces used for children's activities were found on top of the diapering surface. Evidence: The diapering surface has a crack in the pad and is no longer absorbent. Two playhouses were observed sitting on top of the diapering surface.

Plan of Correction: We will replace the pad and will no longer place toys on top of the diapering surface.
