Comments:
An unannounced renewal inspection was conducted on 07/12/2019. There were 10 children, ages 3 years-13 years, totaling 15 point and one provider present during the inspection. The inspector reviewed 5 children's records, one provider record, one substitute provider record, and 2 pet records during the inspection. The provider stated that she does not administer medication. The children were observed in the following activities: having breakfast, in free choice activities, reading and preparing to go outside for a special activity. The inspector reviewed the required the update emergency preparedness plans, the emergency supplies and the outdoor play space during the inspection. The inspector discussed the renewal process, annual training hours and reminded the provider of updating tuberculosis test next month. Standard #: 22VAC40-111-60-B
Description: Based on record review and discussion, the provider failed to ensure that each child's record shall contain all of the elements as required by the standards. Evidence: The record for Child #5 did not contain the following information: the name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached (there was only one contact listed and the address for that contact person was incomplete) and the first date of attendance.

Plan of Correction: The provider will get the missing information from the parent.
