Comments:

An unannounced renewal inspection was conducted today from 9:50am-11:50am. There were 12 children (32 points) directly supervised by 3 staff. The home, 4 staff records, 2 children?s records, 1 household member?s record, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed singing songs, dancing, playing outside, riding in a stroller, pushing toy cars and playing freely. There was a supply of books, toys, and materials for the children. Description:	Based on record review, the provider did not maintain complete children's records. Evidence: Child 1 is missing one parent work phone number.
Plan of Correction:	I will ask the parent to update the form.
Standard #:	22VAC40-111-90-C
Description:	Based on record review, the provider did not obtain once every six months for children under the age of two years. Evidence: Child 2 (age 3 years old) has an immunization record dated 10/3/2016 and no update. (Child 2, DOB 6/6/2016)
Plan of Correction:	I will ask the parents to bring an updated immunization record.
Standard #:	22VAC40-111-180-A
Description:	Based on record review, the provider did not obtain for each caregiver a current report of tuberculosis screening form every two years. Evidence: Staff 2 has a tuberculosis screening which was last conducted on 9/26/2016.
Plan of Correction:	The staff member has an appointment to see their doctor.
Standard #:	22VAC40-111-210-A
Description:	Based on record review, the provider did not ensure caregivers obtained 16 clock hours of training annually. Evidence: Staff 1 and 2 had 0 hours of training in 2018.
Plan of Correction:	We will all get 16 hours of training for this year.
