Comments:
An initial study was conducted from 9:45 a.m. - 11:30 a.m. There were no children in care during the inspection. The family day home was observed for age/stage appropriate supplies, equipment, and activities for the ages of children the provider has requested to care for. The provider, two household members, and one assistant records were reviewed and in compliance. The weather band radio, flashlight, and first aid kit were reviewed. Standard #: 22VAC40-111-770
Description: Based on observation and interview with the provider it was determined that there were no batteries for the weather band radio. Evidence: The weather band radio was observed however, there were no batteries to accompany the radio for it to be operable. Extra batteries are also required.

Plan of Correction: The provider will purchase batteries for the weather band radio.

Standard #: 22VAC40-111-790
Description: Based on observation and interview with the provider it was determined that not all required telephone numbers were posted. Evidence: The following telephone numbers were not posted in the family day home: The telephone number for 911, Poison Control, and the person responsible for emergency backup care.

Plan of Correction: The provider will post the required telephone numbers.

Standard #: 22VAC40-111-330-A
Description: Based on observation and interview with the provider it was determined that a landline telephone was not operable and accessible during the inspection. Evidence: The provider does not have a landline telephone number or landline telephone in the family day home.

Plan of Correction: The provider will contact a phone company today to install a landline. The provider will purchase a landline telephone.
