Technical Assistance:
Discussed the following with the Provider: - storage of breast milk - immunization requirements (including timeframes for them to be completed)

Comments:
An unannounced renewal inspection was completed today. The physical plant, 3 caregiver/household member files, 5 children's files, caregiver training, child nutrition, children's activities, emergency procedures, and supplies were inspected today between 12:40-2:00pm. There were 7 children (plus the Provider's 2 children), totaling 22 points, under the supervision of 2 caregivers. Today, the children were observed being fed a bottle (infant), played outside, painted pictures, enjoyed free play and rested during naptime. The interactions between the children and caregivers were seen to be positive and constructive in nature. Today, the children were served beef kabobs, rice, mixed vegetables and milk. At this time, the Provider is not certified in MAT (medication administration training) and there are no children with any medication needs. However, the caregivers are looking into obtaining this certification. Both of the caregivers have completed over 20 hours of training within the past year. Should you have any questions, please contact me at mary.wizbicki@dss.virginia.gov.

Violations:
Standard #: 22VAC40-111-90-C
Description: Based on file review, 1 out of 5 children's records does not have additional immunization records every six months up to 2 years on file. Evidence 1. Child #4 is 27 months old. The most recent Immunization record was updated when the child was 16 months old.

Plan of Correction: The Provider will ask the parents for this.
