Technical Assistance:
The model form for emergency procedures was reviewed - a copy was provided.

Comments:
An unannounced subsidy health and safety inspection was conducted on 07/12/2019 from 9:50am - 12:15pm. A monitoring inspection was concurrently conducted as this vendor is a licensed family day home provider. There were five children in care with two caregivers. The ages of the children counted for a total of 14 points. Children were observed playing with toys in the playroom. The provider was observed feeding and playing with the two infants in care. Outdoor play and morning snack was also reviewed. Records reviewed for three children and both caregivers.

Standard #: 22VAC40-665-160-B
Description: Based on record review, the vendor failed to maintain a written hard copy record of daily attendance that documents the arrival of each child in care as it occurs. Evidence: 1. The attendance sign-in log for the family day home did not indicate the arrival of child 3 and child 4 for 07/12/2019. a. Child 3 and child 4 were in care during the inspection.

Plan of Correction: This was corrected during the inspection. The parent of child 3 and child 4 signed the children in on the wrong date.
