Comments:
An unannounced renewal inspection was conducted today,July 15, 2019 to determine the provider's compliance with Family Day Home Licensing Standards. The Inspector arrived at 1:20 pm and concluded the inspection at 4:00 pm. Upon the inspector's arrival, nine children (value of 18 points) were in care with two caregivers. Children were observed finishing lunch and playing with toys. Bathroom breaks and hand washing was conducted after lunch. Children watched a video before preparing for nap. After nap children and the provider played a card game of uno. All areas used for child care were inspected along with a walk through of the home. The family day home has age appropriate materials and equipment. The provider interacted with the children in a positive manner. The home's first aid kit and non-medical emergency supplies were inspected and found complete. The provider had the following information posted: license, last inspection notice and emergency phone numbers. The inspector interacted with the provider and children in care. The provider's record, three caregiver/household member's records and nine children's records were reviewed. The provider has current CPR and First. Per the provider, medication is not administered. Transportation is not being provided at this time. The most recent evacuation drill was conducted on 06/28/2019. The two required shelter-in-place drills for 2019 have not been conducted. They will need to be completed by the end of the year. The Emergency Preparedness plan was current. A complete renewal application will need to be submitted and received by the Department prior to the license expiration. Standard #: 22VAC40-111-880
Description: Based on observation and interview, the provider did not follow the most recent, age-appropriate nutritional requirements of a recognized authority such as the Child and Adult Care Food Program of the United States Department of Agriculture (USDA). Evidence: 1. Children were observed eating a jelly sandwich,apple juice, chips and a fruit cup for lunch. 2. Today's lunch did not contain a protein component as required. 3. The provider stated she had not gone to the store over the weekend.

Plan of Correction: Per the provider: I will make sure meals have all components.

Standard #: 22VAC40-111-920
Description: Based on observation and interview, the provider did not ensure when meals or snacks are provided by the family day home, the menu for the current one-week period was given to parents or posted or placed in an area accessible to parents. Evidence: 1. The provider stated she had not made a menu for the week of July 15th through July 19th. 2. There was no menu post.

Plan of Correction: Per the provider: I I will complete the menu today.

Description: Based on review of nine children's records, The provider did not obtain all required information for four children's records. Evidence: 1. The record of child #2, enrolled 09/08/2011, did not have the physical home address listed for the child, name/address/phone number for one emergency contact and the address for the second emergency contact listed as required. 2. The record of child #7, enrolled 12/04/2015, did not have the physical home address listed for the child, name/address/phone number for one emergency contact and the address for the second emergency contact listed as required. 3. The record for child #4 did not have documentation of the parents annual review of the emergency contact information as required. The most recent review was documented on 09/06/2017. 4. The record for child #4 did not have documentation of the parents annual review of the emergency contact information as required. The most recent review was documented on 04/03/2018.

Plan of Correction: Per the provider: I will contact the parents and have them complete the missing information and review the records.
