Comments:
A Subsidy Health and Safety inspection was conducted on July 15, 2019 to review supplemental health and safety requirements. The Inspector arrived at 1:20 pm and concluded the inspection at 4:00 pm. Upon the inspector's arrival, nine children (value of 18 points) were in care with two caregivers. Children were observed finishing lunch and playing with toys. Bathroom breaks and hand washing was conducted after lunch. Children watched a video before preparing for nap. After nap children and the provider played a card game of uno. All areas used for child care were inspected along with a walk through of the home. The family day home has age appropriate materials and equipment. The provider interacted with the children in a positive manner. The home's first aid kit and non-medical emergency supplies were inspected and found complete. The provider had the following information posted: license, last inspection notice and emergency phone numbers. The inspector interacted with the provider and children in care. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. The provider's record, three caregiver/household member's records and nine children's records were reviewed. The provider has current CPR and First. Per the provider, medication is not administered. Transportation is not being provided at this time. The most recent evacuation drill was conducted on 06/28/2019. The two required shelter-in-place drills for 2019 have not been conducted. They will need to be completed by the end of the year. A lock down drill will also need to be conducted. Standard #: 22VAC40-665-150-G
Description: Based on review of children's records and interview, the provider did not maintain in a way that is accessible to all caregivers, a current written list of all children's allergies, sensitivities, and dietary restrictions that was dated and kept confidential. Evidence: 1. The record of child #9 indicated a food restriction. 2. The provider stated she did not have a confidential list of allergies, sensitivities, and dietary restrictions.

Plan of Correction: Per the provider: I will make a list of children's allergies, sensitivities, and dietary restrictions that is dated and accessible to all caregivers.

Standard #: 22VAC40-665-160-B
Description: Based on review of the attendance sheet, the vender did not maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs. Evidence: 1. Eight children were signed in on the attendance sheet while nine children were observed in attendance. 2. Child #8 was not signed in on the attendance sheet.

Plan of Correction: Per the provider: I will make sure children are signed in and out daily.

Description: Based on interview and review of caregiver records, the provider did not ensure caregivers completed Virginia Preservice Training for Child Care Staff sponsored by the Department of Social Services within the first 90 days of employment or service. Evidence: 1. Caregiver #2, caregiver #3 and caregiver #4 did not have documentation of completing the Virginia Preservice Training within 90 days from approval as a subsidy vender dated 02/21/2019. 2. The provider stated that caregiver #2, caregiver #3 and caregiver #4 had not completed the Virginia preservice training.

Plan of Correction: Per the provider: I will have all the caregivers complete the training.

Standard #: 22VAC40-665-400-B-5
Description: Based on review of the emergency preparedness plan, the provider did not ensure the plan contained all components for lock down procedures. Evidence: The emergency preparedness plan did not contain procedures for; Methods to alert caregivers and emergency responders, Methods to secure the family day home and designated lockdown locations; methods to account for all children in the lockdown locations; methods of communication with parents and emergency responders; Accommodations or special requirements for infants, toddlers, and children with special needs to ensure their safety during lockdown; and Procedure to reunite children with a parent or authorized person designated by the parent to pick up the child.
