Comments:
An unannounced Monitoring Inspection was conducted on July 16, 2019 from approximately 10:45 am to 2:00 pm. Upon entrance there were were 12 children, totaling 27 points and 2 providers present. Children were observed freely playing outside, eating lunch and during their rest period. The sample size of records reviewed on this date were 9 children's records, 1 provider record, and 1 assistant record. Areas of noncompliance are identified on the violation notice. Violations:
Standard #: 22VAC40-111-730-D
Description: Based on observation and provider interviews, the provider did not ensure medication, except for those prescriptions designated otherwise by a written physician's order, including refrigerated medication and medications for caregivers and household members, shall be kept in a locked place using a safe locking method that prevents access by children. Evidence: In the first aid kit, there were 4 pain reliever medications. The first aid kit was sitting on a shelf in the kitchen/ dining room area, a designated place for children. The first aid kit was unlocked and the medications were not stored in a locked place.

Plan of Correction: The provider placed medication in the lock box.

Standard #: 22VAC40-111-900-A
Description: Based on observation and provider interviews, the provider did not ensure that in order to assist in preventing choking, food that is hard, round, small, thick and sticky, or smooth and slippery such as whole hot dogs sliced into rounds, nuts, seeds, raisins, uncut grapes, uncut raw carrots, peanuts, chunks of peanut butter, hard candy, and popcorn shall not be served to children under four years of age, unless the food is prepared before being served in a manner that will reduce the risk of choking, i.e., hot dogs cut lengthwise, grapes cut in small pieces, and carrots cooked or cut lengthwise. Evidence: Child #8, age 17 months and child #9, age 22 months were served large chunks of watermelon that measured approximately 2 1/2 inches in length and approximately 1 1/2 inches wide.

Plan of Correction: Provider will cut up watermelon in the future.

Standard #: 22VAC40-111-100-A
Description: Based on review of nine children's records, the provider did not ensure to obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence: The record for child #2 (first date of attendance 9-10-18) did not include documentation of a physical.

Plan of Correction: The provider will make sure the mother gets with the physician to have it.

Standard #: 22VAC40-111-500-C
Description: Based on observation and measurements and provider interviews, the provider did not ensure that stationary outdoor playground equipment shall meet all requirements. Evidence: In the designated outdoor play space there were 3 large truck tires used for climbing and it was within 1 1/2 to 2 feet of a plastic blue and brown slide. The equipment was not placed at least six feet from the perimeter of other play structures or obstacles.

Plan of Correction: The provider will move it to the sanded area.
