63.2 Facilities and Programs

Technical Assistance:
Due to the background check related violations cited in today?s inspection, the provider is required to follow-up with the appropriate department regarding the status of any missing and/or incomplete background checks. Documentation of the completed follow-up must be sent to the Licensing Inspector within 10 business days, and a copy of all missing/incomplete background check results must be sent to the Licensing Inspector upon arrival.

Comments:
An unannounced renewal inspection was conducted today from 10:30am-1:00pm. There were 8 children (25 points) supervised by 2 direct care staff. The home, 2 staff records, 8 children?s records, 1 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing with blocks, singing songs, and engaging freely. Diapering and handwashing procedures were also observed. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, 5 of 8 children's records did not contain documentation of all required information. Evidence: 1. Child #1's (start date: 1/2/2018) record did not contain documentation of proof of identification. 2. Child #2's (start date: 5/13/2019) record did not contain documentation of parent's signed acknowledgement of the receipt of the information required by 22 VAC 40-111-70, and written authorization if a caregiver is to administer prescription or nonprescription medication to the child as required by 22 VAC 40-111-700 A 2. 3. Child #3's (start date: 8/8/2016) record did not contain documentation of parent's signed acknowledgement of the receipt of the information required by 22 VAC 40-111-70, and documentation of the review of the child's emergency contact information as required by 22 VAC 40-111-780 B. 4. Child #4's (start date: 4/23/2018) record did not contain documentation of parent's signed acknowledgement of the receipt of the information required by 22 VAC 40-111-70, and proof of identification. 5. Child #8's (start date: 4/15/2019) record did not contain documentation of parent's signed acknowledgement of the receipt of the information required by 22 VAC 40-111-70.

Plan of Correction: I will print the missing forms and have the parents sign them, and will place them in their files.

Standard #: 22VAC40-111-240-A
Description: Based on observation, all areas and furnishings of the family day home were not maintained in a clean, safe, and operable condition. Evidence: 1. 3 of 4 infant cribs had chipped and peeling paint along the handrails. 2. Outside in the children's outdoor play area, 1 of 3 wooden benches had a wooden board that was cracked and deteriorating. 3. The two large children's play structures on the outdoor play area had visible dirt and spiderwebs over them.

Plan of Correction: I will have the outdoor area cleaned and I will paint the cribs this weekend.

Standard #: 22VAC40-111-680-A
Description: Based on observation, 1 of 2 caregivers did not wash their hands with liquid soap and warm running water after diapering a child. Evidence: Caregiver B (date of hire: 1/10/2019) was observed changing a child's diaper and did not wash their own hands after diapering the child.

Plan of Correction: Caregiver was reminded to wash their own hands and the children's hands after diapering.

Standard #: 22VAC40-111-680-B
Description: Based on observation, 1 of 2 caregivers did not ensure that children's hands were washed with liquid soap and warm running water after diapering. Evidence: Caregiver B (date of hire: 1/10/2019) was observed changing Child #8's diaper and did not wash the child's hands after diapering.

Plan of Correction: Caregiver was reminded to wash their own hands and the children's hands after diapering.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review, 1 of 2 caregiver records did not contain documentation of central registry findings within 30 days of employment. Evidence: Caregiver B's (date of hire: 1/10/2019) record did not contain documentation of central registry results.

Plan of Correction: I will have the caregiver complete the form and mail it next week.
