Comments:
An unannounced renewal inspection was conducted today from 10:50am-1:30pm. There were 5 children (16 points) supervised by 2 direct care staff. The home, 2 staff records, 4 children?s records, 3 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing a color matching game, having circletime and storytime, eating lunch, and transitioning to naptime. Lunch served was ravioli, broccoli, mixed fruit, and milk. Diapering and handwashing procedures were also observed. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, 3 of 4 children's records did not contain documentation of all required information. Evidence: Child #1 (start date: 8/13/2014) and Child #2's (start date: 1/20/2019) records did not contain documentation of a completed physical form signed by a physician.

Plan of Correction: We will ask parents to provide physicals by next Friday.

Standard #: 22VAC40-111-70-B
Description: Based on a review of children's records, 1 of 4 children's records did not contain documentation of parent's written acknowledgement of the receipt of the information in this section. Evidence: Child #3's (start date: 6/2016) record did not contain documentation of parent's written acknowledgement of the receipt of the information in this section.

Plan of Correction: I will have the parents sign it today.

Standard #: 22VAC40-111-320-G
Description: Based on record review, the provider did not ensure that the wood burning fireplace was inspected annually by a knowledgeable inspector to verify that the devices are properly installed, maintained, and cleaned as needed. Evidence: There was no documentation of an inspection for the wood burning fireplace.

Plan of Correction: We will contact someone to come out and do an inspection.
