Comments:
An unannounced monitoring inspection was conducted today from 9:50am-11:20am. There were 9 children (21 points) supervised by 2 direct care staff. The home, 2 staff records, 5 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed cleaning up, having storytime, and playing freely.There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Description: Based on record review, the provider did not ensure that parents were provided all of the required information in writing. Evidence: 1. The provider's written information for parents did not include documentation of the requirement for paid caregivers to report suspected child abuse or neglect according to ? 63.2-1509 of the Code of Virginia. 2. The provider's written information for parents did not include documentation of the address of the department's website, with a note that a copy of this regulation and additional information about the family day home may be obtained from the website, including compliance history that includes information after July 1, 2003.

Plan of Correction: Provider had added missing information into parent form and will obtain parent signatures.
