Comments:
An unannounced monitoring inspection was conducted on 07/12/2019 from 9:50am - 12:15pm. At the time of entrance there were five children in care with two caregivers. The ages of the children counted for a total of 14 points. Children were observed playing with toys in the playroom. The provider was observed feeding and playing with the two infants in care. Outdoor play and morning snack was observed. The inspector additionally reviewed written policies for the family day home. Records reviewed for three children, the provider and one household member. Violations were observed in three parts of the family day home standards and are listed on the violation notice.

Violations:
Standard #: 22VAC40-111-830-C
Description: Based on record review and interview, the provider failed to ensure that documentation shall be maintained of emergency evacuation drills. Evidence: 1. The emergency log for the family day home lacked documentation of a fire drill for June 2019. 2. The provider stated that a fire drill had been conducted in June however, it had not been noted on the emergency log.

Plan of Correction: The June fire drill will be added to the emergency log for the family day home.

Standard #: 22VAC40-111-60-B
Description: Based on record review, the provider failed to ensure that children's records contained all of the required elements. Evidence: The record for child 1 and child 2 lacked a street address for the second designated emergency contact.

Plan of Correction: The parent will be asked to provide the address today.

Standard #: 22VAC40-111-70-A
Description: Based on record review, the provider failed to ensure that children's records contained all of the required written information. Evidence: 1. The enrollment record for child 1 lacked the following: a. Policies for the administration of medications; b. Whether or not there is liability insurance of at least $100,000 per occurrence and $300,000 aggregate in force on the family day home operation as required by ? 63.2-1809.1 of the Code of Virginia. 2. The provider confirmed that this information was not in the record of child 1.

Plan of Correction: The parent of child 1 will be provided with written information regarding medication and liability insurance for the family day home.

Standard #: 22VAC40-111-90-A
Description: Based on record review and interview, the provider failed to ensure that before a child may attend the family day home, the provider shall obtain documentation that the child has been adequately immunized. Evidence: 1. There was no documentation of immunizations on file for child 1. a. Child 1 has an enrollment date of 07/01/2019 and was in care during the inspection. 2. The provider confirmed that immunization information had not been obtained for child 1.

Plan of Correction: Child 1 was taken to his physician yesterday for immunizations. The parent will be asked today to provide written documentation of his immunizations.

Standard #: 22VAC40-111-540-A
Description: Based on observation, the provider failed to ensure that when infant seats are used the safety strap shall be securely fastened. Evidence: 1. The provider placed child 1 in an infant seat without securely engaging the safety strap at approximately 10:20am.

Plan of Correction: The safety strap was fastened. Going forward, the safety strap will be fastened at all times.
