An unannounced monitoring inspection was conducted from 12:00pm-1:15pm. There were 2 children (6 points) in the direct care and supervision of the Provider. The evacuation drills, emergency plan, 2 children's records, 3 Provider/household member records, and the day care space were reviewed. The children were observed sleeping on their cots as it was nap time. The family day home was organized and had an ample supply of age appropriate materials for the children. Standard #: 22VAC40-111-60-A Description: Based on a review of children's records, 1 out of 2 children's records was incomplete. Evidence: 1. Child #2 was missing a name, address, and phone number for 1 of 2 required emergency contacts. Plan of Correction: I will ask the parents to give a second emergency contact.
